Enhancing the continuum of care across the aged care sector, is our growing presence in providing care in peoples’ own homes and in the community across a breadth of services.  This includes services for people with a disability.


Upper Lachlan Community Care is the division of Crookwell/Taralga Aged Care that delivers community based services and services for people with a disability.  This service compliments our residential aged care services.  We are approved providers for:

  • Commonwealth Home Support Program
  • Home Care Packages
  • National Disability Insurance Scheme

Who can receive services?
Every request is assessed individually. If we are unable to provide a service, we will suggest other services that may be able to assist you.

Respite Care
In-home Respite Care is care given as an alternative arrangement with the primary purpose of giving the carer or care recipient a short-term break from their usual care arrangements.

Personal Care
Personal care includes assistance with personal hygiene, washing or showering, dressing and personal grooming.

Domestic Assistance
Domestic Assistance includes assistance with cleaning, dishwashing, laundry and ironing, shopping and bill paying.

Social Support
Social Support includes assisting clients to participate in community life through meeting their needs for social contact and may include assistance with shopping and other related activities and home based social contact.

Transport
Assistance may be given with transport where there is an identified need.

Support for People with a Disability (adults and children)

Home Care Packages are delivered on an individual basis.  Services vary depending on the needs and wants of the person and their budget (package level).

 

Our Staff

Upper Lachlan Community Care is proud of the skills and capacity of all our Home Care Workers who are experienced and appropriately trained.  All our staff have have a minimum Certificate III (or are working towards this), current First Aid Certificate, current and acceptable Federal Police Check and Working with Children Check. Staff also receive training on going personalised training relevant to their position.

 QUALITY OF CARE

Upper Lachlan Community Care is assessed regularly by the Australian Aged Care Quality Agency.  The Agency looks reviews all processes and continuous improvement against the Home Care Standards to ensure all clients are receiving the best possible care.
